Notice to Superior HealthPlan Medicare Advantage (DSNP) Members

Superior HealthPlan learned October 8, 2015 that a former employee downloaded files with members’ protected health information to a personal device. This was against company policy. It happened between March 8, 2015 and March 17, 2015. The employee left the company on March 18, 2015. The information taken included members’ names, Medicaid IDs, Medicare IDs and Superior member ID. In some instances, addresses, dates of birth, and social security numbers were also taken. No credit card or financial information was shared.

Superior contacted the FBI October 25, 2015 who investigated. The FBI ended its investigation on April 5, 2016 and did not find the storage device. They do not believe that member information has been used wrongly. 

Superior takes member privacy seriously. Superior reviewed its security rules and took steps to help prevent this from happening again. This includes updating computer tracking procedures and adding new software. This software will prevent information from being taken without permission.

Superior will notify by mail members affected by this issue. They will be offered free credit monitoring and identity theft protection services.

Superior HealthPlan Advantage is an HMO SNP plan with a Medicare contract and a contract with the Texas Medicaid program. Enrollment in Superior HealthPlan Advantage (HMO SNP) depends on contract renewal.

This plan is available to anyone who has both Medical Assistance from the State and Medicare.
